Documents required for processing of insurance claims are:
- Policy Document, and it is exempted for e-policy
- Company Death Claim Request Form (Part 1) with the signature of the beneficiary
- Original and a copy certified as true of the national ID card of the beneficiary
- Original and a copy certified as true of the house registration certificate of the beneficiary
- Original Death Certificate and a copy certified as true by the benefiary
- Copies of the House Registration Certificate with notification of death of the payer and the payer's national ID card, certified as true by the beneficiary
- The original and a copy (certified as true) of the Identity Card of the payer
- The beneficiary's authorization form to reveal the insured's medical treatment history
- The physician report in Company Death Claim Request Form (Part 2) in case of death at the hospital or medical center
- Other documents, e.g. evidence of name change of the payer or beneficiary, a copy of birth certificate if the beneficiary is a juvenile, a copy of marriage certificate if the benefiary is the insured's spouse
- In cases of unnatural cause of death – e.g. an accident, death from an attack, suicide, unknown causes, etc. – other documents also need to be submitted. These are:
- Copy of Police Report by Royal Thai Police
- Copy of Autopsy Report or Investigation Report on the body where there has been an autopsy, if any
Note: Every document copied must be certified as true by the Insured or the Insured's parents if the Insured is a juvenile.
