1. Death by natural causes such as illness or diseases
The following documents are required:
- Policy contract or certificate of insurance to send back to the company, in case of e-policy, this condition is exempted
- Death Claim Form Part 1 with the signature of all beneficiaries or eligible individuals
- Physician's Report (Death Claim Form Part 2), in case of death at the hospital or the nursing facility
- Copy of the national ID card or other identity documents issued by the Government of all beneficiaries or eligible individuals with certified signatures
- Copy of the House Registration Certificates of all beneficiaries or eligible individuals with certified signatures
- Copy of death certificate from the government, hospital, or any authorized unit with certified signature of the beneficiary
- Copy of the House Registration Certificate with notification of death of the Insured and national ID card of the the insured with certified signature of the beneficiary
- Company's Authorization Form to reveal the medical history of the insured signed off by the beneficiary
- Other documents, such as the Insured and the beneficiary's Name Change Certificate, a copy of the Birth Certificate in the event of the beneficiary being a minor, a copy of the Marriage Certificate in the event of the beneficiary being the Insured's spouse
*Note: All copies of documents must be certified by the beneficiary, or the beneficiary’s parents if the beneficiary is a minor.
2. Death by unnatural causes e.g. by accident, suicide, being attacked by others or animals, or unknown causes
Additional documents, apart from those described in Section 1, are required. These are:
- Copy of Police Report with the certified signature of the authorized official
- Copy of Autopsy Report or Investigation Report on the body, if any, from the authorized government or medical unit with the certified signature of the authorized official
