How to submit an Industry Practices claim

Please include the following information with any claim notification. We’ll also need the contact details of the person in your company who will be dealing with the claim.

  • Insured's name
  • Policy number
  • Date of loss
  • Brief summary of facts and circumstances
  • Copies of any relevant correspondence including letters of demand
  • Copies of pleadings
