Corporate Accident & Health and Travel Claim

For Group Personal Accident / Overseas Secondment Insurance Claims, please download the relevant claim form and submit the completed form to AHClaims.SG@chubb.com
 

Financial Lines Claim

For Financial Lines Claims, claim forms are not applicable. Words in bold have a specific meaning in the Policy and the Policy should be referred to when making any notification.

Please submit the following details / supporting documents (where applicable) for review to Claims.SG@chubb.com:
 

  • Name of policyholder
  • Policy number
  • Date of loss
  • Description of the claim or circumstances which the Insured considers may give rise to a claim or loss, including chronology of events where relevant
  • Nature of the alleged wrongful act, where applicable
  • Nature of the alleged or potential loss
  • Names of actual or potential claimants and their relationship with the Insured
  • Details of when and how the Insured first became aware of the claim or circumstances
  • Copies of letters of demand / pre-action letters / court documents / notices from authorities including copies of all relevant contracts / correspondences / legal opinion etc.
  • Details of any defence lawyers appointed including letters of engagement, hourly rates etc.
  • Any other documents which the Insured may consider relevant to the notified matter

Liability Claim

To submit a Liability Claim, please download the claim form below and submit the completed form and supporting documents (where applicable) to Claims.SG@chubb.com:
 

  • Liability Claim Form

    • Incident report / internal investigation report
    • Police report
    • Colour photographs showing the damage to property or bodily injury
    • CCTV footage
    • Repairer's assessment report on the cause and extend of damage
    • Medical report from third party on bodily injury
    • Letter of demand from third party

Marine Cargo Claim

For Marine Claims, claim forms are not applicable. Please submit the following details / supporting documents (where applicable) for review to Claims.SG@chubb.com:

  • Marine Cargo Claim Supporting Documents
    • Description of loss
    • Commercial invoice (with incoterms)

  • Freight Forwarder Liability Claims Supporting Documents
    • Description of loss and the Assured's involvement

    • Assured's contract with the cargo owner 
    • Bills of lading or airway bill 
    • Estimated loss value / quantity

Property Claim

To submit a Property Claim, please download the claim form below and submit the completed form and supporting documents (where applicable) to Claims.SG@chubb.com:

    • Colour photographs showing the damage

For all employers (excluding platform operators)

To start a work injury insurance claim, please report the work accident to MOM. Visit MOM’s website for more information.

For work accidents that are not reportable, please download the Claim Form and submit the completed form with supporting documents^ (where applicable) to WICAclaims.SG@chubb.com.

For platform operators

To start a work injury insurance claim, please report the work accident to MOM. Visit MOM’s website for more information. Upon receipt of your Chubb Claim Number, proceed to upload any supporting documents^ (where applicable) via our online portal.

For work accidents that are not reportable:
 

  1. Download and submit completed Claim Form to WICA-PWClaims.SG@chubb.com.
  2. Upon receipt of your Chubb Claim Number, proceed to upload any supporting documents^ (where applicable) via our online portal.
For platform workers

In the event of a work accident, please inform your platform operator of the work accident as soon as possible. You should also cooperate with your platform operator and their insurer to provide supporting documents, such as the original medical certificates, medical reports, medical bills, or photographs of the accident to facilitate claim processing. Visit MOM’s website for more information.

 

^ Examples of Supporting Documents:

  • Injured worker’s work permit (for foreign workers) / NRIC (for Singaporean)
  • Medical tax invoices and medical leave certificates incurred by injured worker within 1 year from the date of accident
  • Injured worker’s payslip / salary voucher for the past 12 months immediately before the date of accident
  • Medical report / doctor’s memo / inpatient discharge summary if available
  • Colour photographs showing the accident
  • CCTV footage showing the circumstances of the accident
