Individuals whose coverage has been terminated under their employer’s group insurance policy can apply for individual coverage with Chubb Life within 31 days of their termination date.
A request for conversion should be directed to Canada.ChubbLife@Chubb.com and should include:
- Employee Name
- Employee Email Address
- Policyholder Name
- Group policy #
- Date of termination of employment
